About the role
This is a capacity-building role, not a transport or companion-only role. Every session is planned and delivered with a clear link to the participant's NDIS goals: skill development, independence, social connection, and participation in community life on the participant's own terms.
From October 2026 the NDIS has significantly reduced funding for social and community participation supports. Continued funding depends on providers evidencing genuine skill development against goals. As a Social Inclusion Officer at 4titude you combine warm, respectful 1:1 relationship-building with the discipline to plan, observe and record progress against those goals.
Participant-led delivery
- Deliver 1:1 and small-group community access support following each participant's individual plan and preferences.
- Plan each session with a clear, documented link to the participant's NDIS goals.
- Adapt support in real time to the participant's energy, mood and preferences on the day.
- Travel with and support participants across community settings: cafes, gyms, shopping, public transport, cultural venues, community groups, exercise and hobby pursuits.
Capacity building and evidence
- Apply positive behaviour support strategies and the NDIS Code of Conduct consistently.
- Deliver participant-specific supports as identified in individual support, behaviour, medication or mealtime plans, as briefed before each session.
- Observe, record and report participant progress and capacity-building outcomes in line with 4titude's documentation and NDIS evidencing requirements.
- Communicate with families, support coordinators and other providers in the participant's circle as required.
Safety, safeguarding and compliance
- Follow 4titude's WHS, incident reporting, safeguarding and risk management procedures at all times.
- Uphold NSW Child Safe Standards and 4titude's obligations under the reportable conduct scheme when supporting participants under 18.
- Escalate concerns, incidents or behaviours of concern promptly to the line manager.
Team and documentation
- Contribute to a positive team culture consistent with 4titude's values.
- Attend briefings, debriefs and team meetings as rostered.
- Complete attendance, incident and progress documentation accurately and on time.
- Represent 4titude professionally with families, community venues and other providers.
